Research
This study (CLOZIN) looks at psychotic disorders. These include schizophrenia, schizophreniform disorder, schizoaffective disorder and psychotic disorders not otherwise specified (NOS). The hearing of voices, for example, is common in these disorders. To improve the treatment of psychotic disorders, we investigate which genes predict these disorders in clozapine users. The goal is to predict who reacts well to clozapine. This is done by means of a short visit during which we take a blood sample and conduct a questionnaire. With the use of blood sampling, the DNA can be investigated.

What kind of patients are we looking for?
To investigate this, 4000 patients with mental disorders are required. We are looking for people who:
1) Are diagnosed with schizophrenia, schizoaffective, or schizophreniform disorder.
2) Use clozapine or have used Clozapine.
3) Speak and read the language of the country of the participating center.
4) Are able of making their own decision whether they want to participate in this research.
5) Are at least 18 years of age.

Patients cannot participate if:
1) They are institutionalized in a psychiatric facility under custody.
2) They have Parkinson's disease.

What exactly is done during the visit?
Clozapine users often get their blood checked to monitor side effects. If you participate in CLOZIN, we will take an additional tube of blood (10 milliliters) during one of these blood withdrawals. In most cases, there will not be an additional venipuncture. If this is necessary, this will be discussed first. In addition, a doctor or researcher will conduct a conversation of approximately 10 minutes. In this conversation the participant will be asked about medication. The study saves the data under a code, not under the participant’s name.

Is participation voluntary?
Participation is completely voluntary. If someone decides not to participate, it is not necessary to explain why not. The decision does not affect the received care. If someone decides to participate, it is always possible to consider to withdraw permission at any time without having to tell why.

What do you get in return?
For participation in the research, the participant will receive € 5,- compensation. In addition, the participant contributes to the improvement of the treatment of psychoses.

What are the possible risks?
There is no risk of participation in this research. For example, there is usually no extra blood draw

Who organizes the research?
CLOZIN was founded by a psychiatrist and neuroscientist. Jurjen Luykx, working at the Brain Center Rudolf Magnus that is part of the University Medical Center Utrecht (UMCU). The study is coordinated by Cynthia Pfeifer, MSc, working at the UMCU Psychiatry Department.

Which centers participate in the research?
The CLOZIN research does not only take place in UMC Utrecht. There are several institutions that also contribute to research, including GGZ-NHN, GGZ Rivierduinen, Yulius Centrum, Altrecht, Ludwig-Maximilians-Universität München, Kuopio (Finland), Vincent Van Gogh Instituut en Reinier van Arkel.

To investigate this, 300 patients with mental disorders are required. We are looking for people who:
1) Are diagnosed with schizophrenia, schizoaffective, or schizophreniform disorder.
2) Use clozapine or have used Clozapine.

4) Are able of making their own decision whether they want to participate in this research.
5) Are at least 18 years of age.

What exactly is done during the visit?
For the CLOZIN study we collect 3 blood draws from participants (37.5 ml in total). We try to do this during your routine blood monitoring and usually it does not involve an additional puncture. In addition, a doctor or researcher will conduct a conversation about side effects. The study saves the data under a code, not under the participant’s name.

Every visit takes approximately 1 hour and 20 minutes. The visits take place before the starts of clozapine treatment, between 4 and 12 weeks after initiation, and 6 months after clozapine initiation. In total there are 3 visits.

Is participation voluntary?

Onderzoek
dit onderzoek (CLOZIN) wordt er gekeken naar psychotische stoornissen. Hiermee wordt schizofrenie, schizofreniforme stoornis, schizo-affectieve stoornis en psychotische stoornissen niet anders omschreven (NAO) bedoeld. Bij deze stoornissen is er bijvoorbeeld sprake van het horen van stemmen.
Om de behandeling van psychotische stoornissen te verbeteren doen wij onderzoek naar welke genen deze stoornissen voorspellen bij clozapine gebruikers. Het doel is om dan te kunnen voorspellen wie er goed reageert op clozapine. Dit wordt gedaan door middel van één kort bezoek waarbij een bloedafname en een vragenlijst wordt afgenomen. Met behulp van een bloedafname kan het DNA onderzocht worden.

Wat voor patiënten zoeken we?
Om dit te onderzoeken zijn er 4000 patiënten met psychotische stoornissen nodig. Wij zoeken mensen die:
1) Gediagnosticeerd zijn met schizofrenie, schizo-affectief of schizofreniforme stoornis.
2) Clozapine gebruiken of hebben gebruikt.
3) De Nederlandse taal spreken en lezen.
4) In staat zijn zelfstandig te beslissen over hun deelname aan dit onderzoek.
5) 18 jaar of ouder zijn.

Patiënten kunnen niet meedoen als:
1) Ze onvrijwillig zijn opgenomen onder de ‘inbewaringstellings-regeling’ (IBS).
2) Ze de ziekte van Parkinson hebben.

Wat wordt er precies gedaan bij het bezoek?
Clozapine gebruikers laten vaak hun bloed controleren om zo bijwerkingen in de gaten te kunnen houden. Als u meedoet aan CLOZIN nemen we tijdens één van deze bloedafnames een extra buisje bloed af (10 milliliter). Meestal wordt u dan ook niet extra geprikt. Als dit wel nodig is, wordt dit eerst met u overlegd. Bovendien zal een arts of onderzoeker een gesprek van ongeveer 10 minuten met u voeren. In dit gesprek zullen u vragen worden gesteld over bijvoorbeeld uw medicatie. De studie bewaart uw gegevens onder een code, niet onder uw naam.

Is deelname vrijwillig?
Deelname is compleet vrijwillig. Als u besluit niet mee te doen, hoeft u verder niets te doen. Het is in dat geval niet nodig om uit te leggen waarom u niet wilt meedoen. De beslissing heeft geen invloed op de zorg die u ontvangt. Als u meedoet, kunt u zich altijd bedenken en op elk tijdstip uw toestemming intrekken, zonder dat u hoeft te vertellen waarom.

Wat krijgt u er voor terug?
Voor uw deelname aan het onderzoek ontvangt u €5,- vergoeding. Daarnaast draagt u bij aan de verbetering van de behandeling van psychosen.

Wat zijn de mogelijke risico’s?
Er is geen risico van deelname aan dit onderzoek. U wordt bijvoorbeeld meestal niet extra geprikt.

CLOZIN is opgezet door psychiater en neurowetenschapper Dr. Jurjen Luykx, werkzaam bij het Hersencentrum Rudolf Magnus dat onderdeel is van het Universitair Medisch Centrum Utrecht (UMCU). De studie wordt gecoördineerd door Cynthia Pfeifer, MSc, werkzaam bij de afdeling psychiatrie van het UMCU.

Welke centra doen mee aan het onderzoek?
Het CLOZIN onderzoek vindt niet alleen plaats in het UMC Utrecht. Onderzoek
In dit onderzoek (CLOZIN) wordt er gekeken naar psychotische stoornissen. Hiermee wordt schizofrenie, schizofreniforme stoornis, schizo-affectieve stoornis en psychotische stoornissen niet anders omschreven (NAO) bedoeld. Elk bezoek zal ongeveer één uur en 20 minuten duren. De bezoeken zullen plaatsvinden voordat u clozapine gaat nemen, tussen 4 en 12 weken en na 6 maanden na het starten met clozapine. Voor het onderzoek onderzoeken wij u dus drie keer.
